Surgical Op Notes was set up by three (then)  trainees from the London Deanery to help junior trainees to formulate operative notes

Templates will be uploaded in the next few months, both for elective and trauma surgery from a database of thousands of notes built up over the course of training by Arash Aframian, Paddy Subramaniam, and Farhad Iranpour.  There are books and resources which teach operative setup and procedure but scarce resources to help construct an operative note once the procedure is complete.
